Medically Complex Program

The Incompass Medically Complex Program provides supports to families with children and young adults, up to the age of 25, living at home who have significant cognitive, physical, and complex health care needs.

Program Overview

The goal is to provide comprehensive wrap-around supports, which consists of specialized case management activities that help families integrate the variety of resources and supports they are receiving in order to care for their family member at home.

This program also offers flexible funding to assist the family in the purchase of additional supports and goods not covered by health insurance. Once it is determined that you are DDS eligible, members of our Family Support team will help identify the best path forward.
