Family Support Services

We are a referral and support program for people with developmental disabilities. Our experienced service navigators provide and recommend the appropriate resources that will improve the quality of life and stability within the home and community.

Programs Offered

Department of Elementary and Secondary Education (DESE)

Additional supports are available to families, who are DDS eligible, through the DESE program model. It is designed to provide in-home and community-based therapeutic supports to students ages 6 to 22, who demonstrate a need for services in the hopes of preventing a more restrictive out-of-home residential placement.

Agency with Choice (AWC)

Additional supports are available to families, who are DDS eligible, through the AWC program model. The individual/family designs customized supports based on a vision and need and directs their services in partnership with a qualified Provider Agency. The individual/family selects their support staff and sets works hours and terms of employment.
